PROCESS
1). Complete all sections of form and return to MMB Representative who contacted you.
2). You will be contacted within 48 hours of submission to schedule an appointment time.
**Hours of operation are every 1st and 3rd Wednesdays and Saturdays from 9:00 AM - 2:00 PM** **MMB Men’s Closet is NOT open to the public and clients will be assisted by appointment only! **
Making Men Better Assessment Form
This program was created to assist you achieve certain goals by eliminating barriers, and or threats to your success. We need you to partner with us and be as transparent as possible, recognizing that we will keep your information confidential, and it will only be used for the sole purpose of helping you succeed! Please fill out the form as thoroughly and accurately as possible as it will include your personal and professional goals. Based on this form, we will create a separate plan to identify, and eliminate anything that is keeping you from reaching those goals.
